Documents Required During Internship Application:
- PP.1.2.FR.0021 - Mandatory / Voluntary Internship Application Form - 3 Copies (The form can be accessed through the UNİKYS Homepage.)
- "Provizyon/Müstahaklık Belgesi" from e-Devlet - 2 Copies
- ID Card photocopy - 1 Copy
- Work Accident Notification Form
- Students who will receive payment from the institution during the internship must prepare PP.1.2.FR.0114 Workplace Internship Agreement - 2 Copies.
Internship Application Steps
- Completing all personal information in the PP.1.2.FR.0021 Mandatory / Voluntary Internship Application Form, having it approved by the institution where the internship will be conducted, and signing it by the student.
- Having the PP.1.2.FR.0021 Mandatory / Voluntary Internship Application Form signed by the department internship coordinator (Class academic advisors are the internship coordinators).
- Having the PP.1.2.FR.0021 Mandatory / Voluntary Internship Application Form signed by the Vice Dean responsible for internships.
- After verification by the Research Assistant responsible for internships, all documents must be submitted to the department secretary to be recorded.
Before starting the internship, after submitting the internship documents:
- Before starting and leaving for the internship, students must obtain the Mandatory Internship Form, SGK Employment Entry Declaration, Internship Logbook, Internship Evaluation Form, and Internship Attendance Tracking Sheet from the Research Assistant responsible for internship procedures.
After completing the internship:
- PP.1.2.FR.0018 - Internship Evaluation Form must be filled out by the employer (to be submitted in a sealed envelope).
- PP.5.3.FR.0021 - Internship / Workplace Training Evaluation Survey Form - To be filled out by the employer.
- PP.5.3.FR.0022 - Internship / Workplace Training Evaluation Survey Form - To be filled out by the student.
- PP.5.3.FR.0024 - Student Internship Evaluation Survey Form must be filled out by the employer.
- After the internship logbook is completed, it must be signed and approved by the employer.
All documents must be submitted completely to the Department Internship Committee within the first week of the academic year. Following document submission, the Internship Evaluation Exam is conducted orally in the department meeting room on the date determined by the Department Internship Committee.
